I authorize Hy-Tek Computers (SparkSignal) to debit my bank account as outlined in the payment terms of this agreement for the monthly internet service fee amount + GST based on the plan/term that the customer has chosen at time of sign-up.
I agree to waive any legislative or regulatory requirement for pre-notification.
I/we have certain recourse rights if any debit does not comply with this agreement. For example, I/we have the right to receive reimbursement for a PAD that is not authorized or is not consistent with this PAD agreement. To obtain more information about your recourse rights, you can visit www.payments.ca
This authority is to remain in effect until Hy-Tek Computers (SparkSignal) has received written notification from me/us of its change or termination. This notification must be received at least ten (30) business days before the next debit is scheduled. I/we may obtain a sample cancellation form, or more information on my/our right to cancel a PAD agreement at www.payments.ca
I authorize Hy-Tek Computer Sales and Service Ltd. (SparkSignal) tocharge my credit card above for the agreed upon monthly service fee + GST for internet services on a recurring basis every month. I understand that my information will be saved on file for future recurring transactions on my account.
You authorize regularly scheduled charges to your credit card. You will be charged the plan amount you have chosen for each billing period plus GST. A receipt for payment will be provided to you via e-mail and the charge will appear on your credit card or bank statement. All invoices are issued on the 1st of each month for the previous month charges (backwards billed). Payments are processed on same date of issuance (1st).
I understand that this authorization will remain in effect until I cancel it in writing and I agree to notify Hy-Tek Computer Sales and Service Ltd. in writing of any changes in my account information or termination of this authorization at least 15 days prior to the next billing date. I certify that I am an authorized user of this credit card and will not dispute these scheduled transactions; so long as the transactions correspond to the terms indicated in this authorization form.

Site Survey Fee = CAD $150 + GST
The installation fee, if applicable, will be due once install is complete.
